Hospital Readmission Rates
Overview:
This report allows you to better identify trends in readmissions to hospitals in and out of your network in real-time. Dive into patient-level detail to conduct root-cause analysis on preventable readmissions to drive and track performance improvement initiatives.
Key Definitions:
- Hospital Readmission Rate: The formula for this calculation is (Hospital readmissions / Total hospital discharges)
- Hospital Readmissions: The total number of admissions within 30 days of discharge from initial hospital admission. The readmission will be counted retrospectively against the month of the initial hospital discharge, i.e. the “anchor admission.” For example, a patient discharged in August and readmitted 25 days later in September will be counted as an August readmission.
- Total hospital discharges: The total number of hospital discharges during the reporting month
- The month of Initial Hospital Discharge: The month during which the initial hospital discharge occurred
- Days to Readmission: The number of days between the initial discharge date and the readmission date
- Initial Hospital: The hospital where the initial discharge occurred
- Initial Admission Date: The date of the initial hospital admission
- Initial Discharge Date: The date of discharge from the initial hospital admission
- Intermediary SNF (as applicable): The SNF facility to which the patient was discharged from the initial hospital. This will be blank if the SNF stay did not begin within 24 hours of discharge
- Readmission Hospital: The hospital to which the patient was readmitted within 30 days of discharge from the initial admission
- Readmission Date: The date of the patient's readmission
- Care Partner(s): Organizations from the Pings community participating in the care of this patient, identified by their inclusion of this patient on a roster submitted to Bamboo
Skilled Nursing Facility (SNF) Care Collaboration

This report allows you to manage SNF utilization, the average length of stay, and associated hospital readmission rates by facility, down to the patient level. Streamline your regular performance reviews and relationship management with your SNF partners by acting on this shared, real-time data.
Key Definitions:
SNF Readmission Rates Report:
- SNF Readmission Rate: The formula for this calculation is (Readmissions / Total SNF Admissions)
- Readmissions: The total number of hospital admissions that occurred within 30 days of an initial inpatient admission AND after discharge from an SNF. Please note that encounters are included only if the SNF stay began with 24 hours of discharge from the initial inpatient admission. The readmission will be counted retrospectively against the month of the SNF admit date
- Total SNF Admissions: The total number of SNF admissions that began within 24 hours of an initial hospital discharge
- The month of SNF Admission: The month during which the patient was admitted to the SNF
SNF Average Length of Stay (ALOS) Report:
- SNF Average Length of Stay (ALOS): The SNF ALOS for patients included on your organization's roster(s)
- The month of SNF Discharge: The month during which the SNF discharge occurred
- Total SNF Discharges: The total number of SNF discharges
SNF Admission Volume Report:
- Total SNF Admissions: The number of SNF admissions for patients included on your organization's roster(s)
- The month of SNF Admission: The month during which the SNF admission occurred
Patient-Level Reports:
- Days to Readmission: The number of days between the initial hospital discharge date and the readmission date
- Initial Hospital: The hospital where the initial discharge occurred
- Initial Admit Date: The date of admission to the initial hospital
- Initial Discharge Date: The date of discharge from the initial hospital admission

Multi-Visit Patients
Overview:
This report identifies patients with the highest hospital utilization (ED & inpatient) and volume trends to most effectively direct your care management resources in real-time.
Key Definitions:
- Patient Count: The number of unique patients included in each hospital visit count category.
- Hospital Visit Count: The number of hospital visits your patients experienced during the selected date range filter. Visits involving observation events have been excluded.
- Hospital Visit Count Category: The number of hospital visits your patients experienced in the selected date range filter. Hospital visit count categories include 2 visits, 3-4 visits, 5-8 visits, and 9+ visits. Visits involving observation events have been excluded
- Your Care Programs: The care programs your organization has specified in its roster submission to Bamboo Health
